By signing below, I agree to assume full responsibility for any and all risk for my child’s participation in this voluntary field trip. I agree not to hold CVCU, its employees, volunteers, subsidiaries, university campuses, or church partners liable for any damages or injuries caused during this trip. I understand that the CVCU Student Code of Conduct regulations apply while students are on the trip and that these behavioral expectations will be enforced. Students who do not follow the Code of Conduct may receive disciplinary action, including expulsion from the program.

In the case of a medical emergency during which the parents cannot be reached, CVCU administration requires the following information to be on file.
